Food Health Collective provides nutrition and food-as-medicine services to eligible healthplan members and patients. Individuals who opt in to our SMS program may receive text messages related to their participation in Food Health Collective services, including appointment reminders, nutrition program updates, service and delivery notifications, and member support communications.
Consent to Receive Messages
By opting in to receive SMS messages from Food Health Collective, you consent to receiverecurring text messages at the mobile telephone number you provide.Consent to receive text messages is not a condition of receiving Food Health Collective services.

Message frequency varies depending on your participation in Food Health Collective services and your communication needs.
